(C-HRT) SAFE HAVEN

C-HRT Safe Haven serves up to 22 unhoused residents living with chronic substance use conditions and is a partnership between the County of San Diego, the City of San Diego, Family Health Centers of San Diego, and Episcopal Community Services.

How C-HRT Safe Haven Works
The City covers facility costs, the County is responsible for operating costs, and we oversee and facilitate the day-to-day operations of the safe haven.

Clients are placed by Family Health Centers of San Diego (FHCSD) for transitional housing support prior to exiting to permanent housing. FHCSD provides ongoing care coordination, case management, and support services as needed.

ECS Leadership, C-HRT Staff, Family Health Centers of San Diego Staff, and County of San Diego Health and Human Services staff receive the Outstanding Behavioral Health/Mental Health Program Award at the Healthcare Heroes Awards. May 2023.
